9. Other Services Provided by NZBS

9.10 Setting Up New Transfusion Facilities

NZBS will consider requests to make blood components or products available for infusion at a facility that has not previously had this service. An application form must be completed and forwarded to NZBS at least three months before blood or products are required, to allow time to ensure all required arrangements for supply.  If new refrigeration equipment is required, lead times may be longer.

NZBS documents that will be useful in this context are:

  • 136D021 Process Diagram of Response to Request for New Transfusion Facility – an overview of the whole process as it affects many departments.
  • 136F213 Application to Receive and/or Store Blood Components or Blood Products – a formal application form for the requesting health provider organisation to complete.
  • 136M168 Request for New Transfusion Facility – a Standard Operating Procedure for Blood Bank to follow.
  • 108I042 Refrigeration Guidelines. Requirements for the Storage of Blood, Blood Products and Tissue for DHB Blood Banks.
